Loosening Implant Failures

Zirconia Abutment
A patient's report of a loose implant can be a relatively minor issue that might involve a loosening abutment or a simple cementation process that has gone awry. It can also be a symptom of something that might be an alert that something more serious might be underway which really should be investigated immediately.

Preliminary Diagnosis ASAP

As a general rule, treating any kind of loose tooth symptom shouldn't be delayed any longer than necessary. It is difficult to predict what events will occur or are occuring elsewhere in the dentition.

An implant that feels like it is loosening, rotating or wiggling around could be a sign of an osseointegration failure or an infection issue. Unknowingly this could prove to be disastrous for surrounding tissues.. if indeed the implant itself is loose.

An implant that is healthfully and well osseointegrated into the jawbone provides all of the stimulative qualities of natural teeth.... the hallmark of dental implants.

When the implant device is NOT well osseointegrated... for whatever reason... the physical forces associated with nearly any movement, especially with eating and chewing no longer provide the stimulative qualities sought when the implant was first installed.

Primary Benefit becomes Trauma Source

Most traditional dental implants can require anywhere from 3 to 9 months to heal well and provide the rock solid stability dentists want before the implant is "loaded" (fitted with a tooth, denture, bridge, etc.). Anything less is not acceptable for obvious health reasons.

Immediate Function, same day or immediate load implants are hybrid implant devices that are designed for implant sites that provide ideal bone and tissue conditions which do not require the usual "healing - osseointegration" period.

Taking the position, momentarily, that osseointegration is failing.... it is critical to remove the implant so the ongoing trauma source can be eliminated. Steps can then be taken to restore the immediate area... which might involve addressing an infection issue, a grafting failure or perhaps the issue of the wrong sized or wrong type of implant for a given depth and density of bone.

Too Many If's

Considering the investment of time, financial resources and a patient's history of treatment sessions, the common urge we all have to delay a trip to the dentist is something to be avoided.

Too many "what if this.... what if that" variables.

The mere threat of a possible loss of the fused bone-to-implant state (osseointegration) that can undo all that we as patients have done to obtain the advantage of the Gold Standard of Tooth Replacement procedures will not only be lost for the implant in question but also for overall bone health.

Persons with multiple implants that are configured to replace several teeth may need to be extra vigilant about the symptom of a loose implanted tooth.

Loose Implant Remedy Options

A loose restoration may be fixed simply with redoing a failed cementation or replacing a failed abutment device. Depending on the implant type, it might involve replacement of a faulty threaded post. A failure of the dental porcelain may require the fabrication of a new crown.

Diagnosed early enough, a failure caused by a compromised grafting treatment might be repaired by a healthier or more robust grafting procedure. Implants that are deemed to be undersized can be replaced with a larger implant, in terms of diameter, thread depth or implant length.

In more complex situations, a loosened implant may require a combination of two (2) or more procedures outlined above.

Precision Diagnostics

In our practice, Dr. Marinic uses a Galileos Cone Beam Scanner that provides advanced 3-D imaging scans that are known to be unsurpassed in generating the technical and imaging data needed for accurately determining the cause of an implant becoming loose.

In situations where generalized occlusal forces are suspected be a causal factor or represent an on-going event, procedures can usually be implemented immediately as a temporary control while a sensible treatment option is developed.
