Objective Osseointegration Assessment

Dental implants are commonly regarded as the Gold Standard of tooth replacement.

Under true ideal circumstances, implant surgery is unsurpassed for treating an individual tooth and is equally unsurpassed for creating complete, full mouth reconstruction that can incorporate any number of implants.

Ideal Treatment for Non-Ideal Circumstances

It is safe to say however, that for most patients, truly ideal pre-treatment oral health conditions probably don't exist. The need for tooth replacement usually comes about due to oral health problems that can include, for example, developmental growth issues, disease, infection, physical trauma and every day wear and tear, to name a few.

Jawbone integrity and overall stability of the upper and or lower jaw bones (depending on treatment need) are the most critical factors in determining candidacy for treatment, along with several other factors.

Most candidates for dental implants have varying degrees of oral health conditions that are carefully assessed by the implant dentist, oral surgeon or implant periodontist. Xrays, panographs, occlusal analysis, traditional catscans and the newer cone beam ctscans are used, as needed, for diagnosing the pre-treatment conditions.

Because implant success is nearly totally dependent upon the quality and overall health of jaw bone material (optimal porosity and blood supply), obtaining an accurate assessment is the major focus of preliminary diagnostic procedures. Patients who have been declared non-candidates typically have a complication involving the aforementioned criteria.

Implant failures nonetheless do occur despite having a prerequisite of heathy bone. Indeed, faulty site preparation surgery can even cause bone loss in the healthiest bone imagineable.

Outside of an active disease process, a frequent cause of failed implantology is due to osseointegration problems. Poor integration of the implant into surrounding bone can be caused by blood supply problems, necrosis of the bone (caused by even more factors), and most notably, an implant loading (placing the final restoration - crown or bridge) that occurs too early.

The average traditional implant typically requires 3-6 months for the implant(s) to become fully integrated with the jawbone. Until recently, the method used by a dental surgeon, periodontist or maxillofacial surgery specialist might include visual inspection, following a chronological time table, xrays, radiographs and/or possibly catscans.

Despite frequent monitoring using any number of these historical assessment methods, implant failures are still known to occur. Many dental professionals will not attempt a repair of a failed osseointegration.
